KY3 Community Coalition met today at the Taylor County Extension Office where a panel made up of acute care, home health, EMS, social work and hospice, had focused discussions around social drivers of health (SDoH) and resources located within the community.  Medicaid MCOs were present to talk about their resources as it relates to transportation and how to connect with their case managers to address patient needs. 

The community learned of a free state government service, kynect resources, which is a website of local programs and services to help community members find assistant with housing, food insecurity, employment, finances and so much more. Another great aspect about kynect resources is they work to close the loop on referrals made out to the organizations, which is vital to meeting the need of the patient.  Community members were encouraged to visit the kynect resources website to learn more: https://kynect.ky.gov/resources/s/?language=en_US  

A similar resource shared during the meeting was www.kyloop.org, which takes folks to the Kentucky Voices for Health KY Loop landing page with many choices presented to choose from.  

Morgan Bray and Scott Thompson gave an update on the Food is Medicine Initiative that Kentucky Hospital Association has partnered with the Kentucky Department of Agriculture on.
